Who we are
This website is operated by [LEGAL ENTITY NAME, PLLC], doing business as Xenith Health (“we,” “us”). Our office is at [STREET ADDRESS], Amarillo, TX [ZIP]. Privacy questions can be sent to hello@xenithhealth.com or [PHONE].
This Privacy Policy covers information collected through the website. Protected health information created or received when you become a patient is governed by our Notice of Privacy Practices under HIPAA and Texas law. Where the two overlap, the Notice controls.
Information we collect
Information you give us
- Consult requests. Name, email, phone, the service you are interested in, preferred contact method and time, and any message you include. Please do not include medical details in the website form; your physician collects your health history during your visit.
- Contact form and email. Whatever you choose to send us.
- Bookings and patient portal.In-office appointments and account login are handled by Boulevard (blvd.app). Information entered there is governed by Boulevard’s privacy policy as well as our agreement with them.
Information collected automatically
- Standard server logs: IP address, browser type, pages visited, and referring URL, kept for security and troubleshooting.
- Cookies or similar technologies used by our hosting provider and by embedded third-party tools (for example, the Boulevard booking widget) to keep the site working.
We do not run advertising pixels or analytics trackers on intake, scheduling, or patient-portal pages. If we add analytics elsewhere on the site, we will update this policy and configure them to avoid collecting health information.
How we use information
- To respond to your consult or contact request and schedule care.
- To operate, secure, and improve the website.
- To send service messages about your request or appointment. We do not sell your information and we do not send marketing texts or emails without your consent.
- To meet legal, regulatory, and payment-processor obligations.
How consult requests travel
When you submit the consult form, your entry is sent to our care team by email through a transactional email service (Resend). If email delivery is unavailable, your device’s mail app may open with the request prefilled so you can send it yourself. Email is not a secure channel for medical information, which is why the form asks you to leave health details for your visit.
Who we share information with
- Service providers that host the website, deliver email, and run scheduling and payments. They may only use your information to provide those services to us.
- Your care: once you are a patient, information needed for treatment, payment, and operations is shared as described in the Notice of Privacy Practices, including with King Compounding Pharmacy for prescriptions.
- Legal: when required by law, subpoena, or to protect the rights and safety of patients, staff, or others.
Your choices
- You can ask us to correct or delete website contact information by emailing hello@xenithhealth.com. Medical records are retained as required by Texas law and are handled under the Notice of Privacy Practices.
- You can decline cookies in your browser; parts of the booking widget may stop working.
- Reply STOP to any text or use the unsubscribe link in any email to opt out of non-essential messages.
Security
The website is served over HTTPS. We limit who can see consult requests and review our safeguards regularly. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.
Children
Our services are for adults 18 and older. We do not knowingly collect information from children through this website.
Changes
We will post any changes here and update the “Last updated” date. Continued use of the site after a change means you accept the updated policy.
